An old ophthalmologist came to me last night when was my duty. He complainted that why he always found patients’ left eyes were in pathologyic changes. He was alert about that. He altered his right eye to check the patients’ left eyes and didn’t find anything wrong. He finally found his left eye was in trouble. He felt scotoma, dysmorphopsia and visual acuity decrease. I dilated his left eye and check the fundus with 90D fundus lens. I found a lot of yellow-white exudates and macular edema in central macular zone. I considered the pathologyic changea ware central exudative chorioretinitis or macular degeneration. I suggested him to come to me the other day to take a FFA exam and fundus OCT to make the diagnosis identified.
Bullshit: If you want to have kidney transplantation, you have to remove your homolateral eyeball for some reasons.

I was on clinical outpatient today. I met three people with complain of eye lids swelling and pain. Signs: eye lids congestion, edema, touch pain and clusters of yellow-white herpes on eye lids. One of them presented a big ulcer on the right corner of the mouth. Same diseases occured in these three patients. They were infected by virus. The most common virus that most of us would suffer at least one time in life time is herpes simplex virus. So I gave anti-virus ophthalmic ointment to them. And told them to take a good rest, eat more vegetables and fruits. Not to stay up late in night, no smoking and wine. Anti-biotic ophthalmic ointment should be dosed if there are signs of sencondary infection.

Oops, allergy appears in my hands caused by sterile water
I don’t konw why my hands appear allergy after applying sterile water before surgeries. I do the same thing for a decade. But recently, my hands feel itch, a little pain and papula arise. Perhaps I have to take pills to adjust fine changes in my body.

Traffic accident leads to both eyes blind and left rupture of the eyeball
A 15 years old boy with traffic accident injury was sent by his uncle to me. The boy with compound cranial&facial fractures and leakage of cerebrospinal fluid was transfered from emergency department and still conscious . I slightly opened the both eyes. I found the right eye with no light perception, severe conjunctiva edema, intact cornea, existing anterior chamber, dilation of the pupil, and no red reflex of the fundus. The CT showed bulk hemorrhage and perhaps the blood was from the sub choriod. The left eye was not seen intactly. Uveal coat was exposed outside of the conjunctiva capsule. I performed urgent surgery on this boy. The surgery was well done. But what left to him was a blind world.

Tomorrow I am on optometry duty

I was very busy the day before yesterday. I got up early to hospital then at 16:30 was my night duty. I got to bed at 2:00 yesterday and got up 6:00 yesterday. I just only had been in sleep for 4 hours in 48 hours.
Emergent patients were:
Two eye lids skin lacerations, a contusion, a conjunctival laceration, four conjunctivitis, a severe keratitis of both eyes, a alkali burns and a trichiasis.
I was exhausted when I finished those surgeries and treatments. It was 2:00 AM then. I was awaked by a patient with acute iriditis. After that, I went to inpatient for surgeries arrangement. There were three surgeries, a vitrectomy for diabetic retinopathy(stage Ⅵ) and two retinal detachments. When I stepped out from surgery room was 15:20 PM. Then three patients need consult from obstetrical department and endocrine department. And I had three new patients of my own in inpatient. I was crazy then. When I finished all the task and had fast food, it was 20:00 PM. Today I have a lecture to student. And tomorrow I will be on optometry duty. Too busy every day.

Today I am going to give a lecture to students in campus today. It is about eye lids, conjunctiva, cornea and sclera diseases. This is my first time. Hope be successful.
