As we know, glaucoma is a blind leading disease. The crowd people of glaucoma have more or less psycho manifestations,such as anxious, insomnia, tension, and sensitive. Some of them take diazepam to help them to sleep or sleep well. But do you know, diazepam would increase the intraocular pressure and accelerate the process of glaucoma. So if you are taking diazepam or some kind of these medicine, quickly drop it and consult your doctor. Or your visual acuity left would lose. Have a good sleep quality means a significance.

How many years can I put on contact lens?
Less than 10 years. If you wear more than this period, contact lens would hurt your ocular surface enviroment, such as lacrimal film, cornea, allergy. So you would always feel itch, red eye, dry eye, blur vision, excretion increase and lacrimation etc. Sometimes you might feel normal even though the contact lens have do harm to you, this is because it would decrease your sensation of your eyes. If you appear those clinical manifestation, go to your ophthalmologist for a advise.

I was on clinical outpatient today. I met three people with complain of eye lids swelling and pain. Signs: eye lids congestion, edema, touch pain and clusters of yellow-white herpes on eye lids. One of them presented a big ulcer on the right corner of the mouth. Same diseases occured in these three patients. They were infected by virus. The most common virus that most of us would suffer at least one time in life time is herpes simplex virus. So I gave anti-virus ophthalmic ointment to them. And told them to take a good rest, eat more vegetables and fruits. Not to stay up late in night, no smoking and wine. Anti-biotic ophthalmic ointment should be dosed if there are signs of sencondary infection.

Right eye blur for a year after a beat by the fist when box trainning
A 20 years old boy who is a boxer occasionly found his right eye could not see clearly when covered the other eye in one morning. He came to us with his right eye blur for a year after a beat by the fist when trainning. For a year, he felt no eye pain, red eye, swelling, omiting and so on. I examed the fundus with three mirror contact lens. And found the retina was nearly detached from the inner eye wall, and with server proliferation. The proliferation zone reached as far as to the ora serrata. The crystal lens was also a little opacity caused by the retina detachment. So I performed vitrectomy on him. Since the retina was in heavy PVR, three big hole were found in the peripheral retina. This was the typical presentation of ora serrata truncation. The peripheral retina incision was inevitable. The aim to do this was to preserve some central visual acuity for him. And I supposed his sport career was terminated after this surgery. Silicon oil was injected to support the retina to recover and adhere to the choroid.

A middle age man came to me with urgent anger closure glaucoma. The intraocular pressure(IOP) was very high, over 60mmHg and the axis length was 20.84mm. I dosed mannitol and dexamethasone to him. Three days later, the IOP was getting down to 16mmHg. I operated the trabeculocanalectomy on him. The anterior chamber depth was getting shallower and shallower when I incised the trabecular meshwork. The iris rushed into the incision and crystalline lens bulged to cornea. The viscoat was used to maintain the anterior chamber depth. I promptly closed the incision. Mannitol and dexamethasone 7mg were used to decrease the pressure of the posterior segment. The anterior chamber depth was getting shallower and shallower day by day after surgery and crystalline lens bulged nearly to cornea. If the anterior chamber depth continues to get shallowed or vanished, lensectomy and vitrectomy is inevitable.
I seldom met such circumstance.

Oops, allergy appears in my hands caused by sterile water
I don’t konw why my hands appear allergy after applying sterile water before surgeries. I do the same thing for a decade. But recently, my hands feel itch, a little pain and papula arise. Perhaps I have to take pills to adjust fine changes in my body.

Four ocular anterior segment analysis intruments
1. Pentacam introduced by Oculus
Equipped with an automatically rotating Scheimpflug camera the Pentacam performs a complete measurement of the anterior eye segment in less than 2 seconds. A precise analysis of the central cornea is carried out in the process. During measurement extraneous eye movements are detected with a second pupil camera and automatically corrected during the calculation process. The software analyses and evaluates all the data acquired. These data then provide the basis for a three-dimensional model of the complete anterior eye segment.
The Pentacam analyses the entire cornea, anterior chamber and crystalline lens. This includes an objective determination of the central radii, corneal asphericity, various coloured maps of curvature and elevation, chamber angle, chamber volume and chamber elevation as well as lens transparency. Tomography shows the individual levels of the anterior eye segment in a freely rotating model. Measurement is non-contact and entirely agreeable to the patient.
2. Orbscan
The Orbscan IIz topographer analyses the physical shape/contours of your cornea and allows a surgeon to decide if it has a suitable shape, is healthy and thick enough for treatment.
It is the only topographer currently available that measures the shape of both the front and back surface of the entire cornea (other systems only measure the front surface) and can therefore provide a complete picture of the thickness of cornea. The Orbscan IIz is able to provide such high precision information as it captures data within seconds from 9000 points on the cornea during each measurement.
3. UBM
The abbreviation of ultrasound biomicroscopy. It is a new technology that uses very high frequency ultrasound (20~55 MHz or even higher, compared to 3~15 MHz in conventional clinical ultrasound systems) to non-invasively image the small animals in biological research. The spatial resolution of a two-dimensional image is up to ~50 μm, with penetration depth of ~20 mm. The functional modalities include B mode, M mode and Doppler mode, etc.
